Covid-19 Office Protocol

What you need to know before booking your next dental appointment

We are open for all dental services. Our team continues to implement and exceed all COVID-19 health and safety standards for hygiene and sanitization, as well as considering every step of treatment processes to ensure we meet your needs in a focused and safe manner as directed by the Ontario Ministry of Health, Peel Public Health, and the Royal College of Dental Surgeons of Ontario (RCDSO). The health and safety of our patients and staff have always been a top priority.

The important changes we continue to practice to keep you and our staff safe:

BOOKING YOUR APPOINTMENT

  • Emergencies: If you have a dental emergency, feel free to contact us at anytime and as always you can count on us to get you in as soon as possible.

  • We pre-screen each patient on the date of booking and on the scheduled date of your appointment. A COVID-19 screening questionnaire will be performed at the time of booking. We ask our patients to refrain from visiting the clinic if you have any of the following COVID-19 related symptoms or feeling unwell:

    • New onset of cough or worsening chronic cough

    • Shortness of breath

    • If adult > 18 years of age: unexplained fatigue/lethargy/malaise/muscle aches (myalgia)

    • If child < 18 years of age: nausea/vomiting, diarrhea

  • If you test positive for COVID-19 in the past 10 days or have been told they should be isolating, please let us know.

  • If you are feeling unwell before or on the date of your scheduled appointment, please give us a call in advance at so we can reschedule your appointment for a future date.

  • We encourage all patients to complete intake forms online to minimize contact prior to the scheduled appointment by downloading the forms from this page. You can either fill out the forms:

    A) Digitally using a PDF software such as Adobe Acrobat and email the completed version to smiles@905dental.ca (preferred option to minimize contact); or

    B) Print and complete the forms at home and bring them to your appointment; or
    C) Printed forms could be provided and completed at our clinic before your appointment. However, please arrive 10 minutes prior to your scheduled appointment for this option.

  • If you experience any symptoms of COVID-19 within 14 days after your visit, please call and inform our office.
